One card per provider. Where a vendor does not publish a figure, the card says so and leaves the gap open.
Reading the table
The score sums up collected ratings and carries no information about price. A cheap provider can sit near the top and an expensive one near the bottom for the same reason.
Address types and coverage are lifted from each vendor own site without rounding or filling in. The gaps you see are gaps they left.
Two figures decide a purchase and almost nobody publishes them: how many distinct subnets come in a set, and what happens when the traffic allowance runs out.
